PLEASE NOTE: After the form has been filled in and, if you have the ability to "sign" on-line, you have the option of saving the form and forwarding it to the ECB Registrar via email as an attachment. The form will be filed and faxed back to you.
If you do not have the ability to "sign" on-line, please print the form after you have filled it in and forward it to the ECB Registrar by mail, courier or fax.
If you want original documents for your files, this process can be followed up by forwarding the original and at least two copies in the mail. The Registrar will file these documents with the same filed date that the documents were received by email or fax.
